Required Health Forms
New students are required to meet certain health-related requirements prior to arriving on campus for the start of the semester. Please read the following information closely.
Immunization Requirements
All new students must submit up-to-date immunization documentation via the College’s Student Health Portal by Friday, June 28, for fall enrollment. As immunization records are required by state law, your student account may be placed on HOLD until your immunization record is received, and Health Services confirms that required immunizations are up to date.
Required Immunizations
The following mandatory immunizations are required by the Massachusetts Department of Public Health (DPH).
Two doses of the MMR Vaccine per appropriate dose schedule are required. Provide immunization documentation or copy of laboratory test/proof of immunity.
One dose of the Tdap Vaccine within the past 10 years is required. Please consider a booster to maintain compliance during all college years.
Three doses of the Hepatitis B Vaccine per appropriate dose schedule are required. Provide immunization documentation or copy of laboratory test/proof of immunity.
Two doses of the varicella vaccine per appropriate dose schedule are required. Provide immunization documentation or copy of laboratory proof of immunity or documentation of history of varicella disease (chickenpox) verified by healthcare provider.
One dose of the Meningococcal Meningitis Vaccine (Men ACWY or MCV4) is required for all full-time students 21 years of age or younger. The vaccination must be on or after the student’s 16th birthday. Doses received at younger ages do not fulfill this requirement.
Other Recommended Immunizations
For the health and safety of our community, Stonehill College Health Services recommends strongly that all students receive these additional vaccines.
The College strongly recommends that all students enrolled in on-campus classes and/or conducting research in on-campus labs, whether they live on campus or off, be “up to date” with their COVID-19 vaccinations, with a primary series of COVID-19 vaccination as well as a booster dose.
The Men ACWY or MCV4 vaccine provides protection for only 4 of the 5 serogroups of meningitis (A, C, W, and Y). Please ask your healthcare provider about receiving the Meningitis B vaccine. The Meningitis B vaccine is not required by the state of Massachusetts, but is the only way to gain protection against meningitis serogroup B. We are promoting the message of awareness: “It Takes Two” vaccine types for full protection against this dangerous disease.
An annual Influenza Vaccine is strongly recommended for all students. The College hosts an annual Flu Vaccine Clinic every Fall, so please look for notifications in September regarding these Vaccine Clinics on campus.
Immunization Exemptions
In accordance with Massachusetts School Immunization Requirements (105 CMR 220.00), requests for medical exemptions may be granted if there is documentation from a healthcare provider stating the reason(s) that these vaccinations are contraindicated. Requests for religious exemptions also may be granted in accordance with Massachusetts state law. In the event there is an outbreak of any of these vaccine-preventable diseases, students who have been granted exemptions may be required to leave campus and will be allowed to return only when the period of communicability has passed.
One dose of the Meningitis vaccine (Men ACWY or MCV4) is required for all full-time students. The vaccine must have been received on or after the student’s 16th birthday. The law provides an exemption for students who sign a waiver that reviews the dangers of meningococcal disease and indicates that the vaccination has been declined. Please note, if a student is under 18 years of age, a parent or legal guardian must be given a copy of this documentation and must sign the waiver.
The Meningitis Waiver should be filled out only if the student declines receiving the vaccine and after reviewing the waiver information.
Additional Health Requirements
A copy of the summary report from your most recent physical exam is required and must be submitted to Stonehill Health Services via the College’s Student Health Portal.
Please note that Stonehill College does not require a new appointment with your healthcare provider (PCP) for submission of the physical exam summary report. While it is required that you submit a copy of your latest summary report, it is not required that you have a specific appointment for college entry. However, you may need an appointment to receive certain vaccines if you are not already compliant with Massachusetts requirements for college entry. Please consult with your pediatrician’s office as soon as possible.
Per state law, all students must complete the Tuberculosis Risk Questionnaire. The questionnaire can be found in the College’s Student Health Portal. If you screen as high risk, your PCP must complete the Medical Evaluation for Latent Tuberculosis Infection form, which is also available in the Health Portal.
The Health Report Form is the required intake form for the Office of Health Services. It asks for your consent for medical care at Stonehill College, as well as questions about your personal health history, medications you are taking, and allergies you might have, and the health history of your family. The form also asks for your consent for medical care at Stonehill College. Please find the Health Report Form in the College’s Student Health Portal.
How to Submit Required Health Forms
Please submit all immunization records and health forms at once, if possible, through the College's Student Health Portal.
Your credentials for the Student Health Portal are the same username and password you use to access myHill, eLearn and other College resources.
Health Services
Stonehill Health Services provides free* and confidential, appointment-based medical care for full-time Stonehill College students during the academic year. Health Services is staffed by nurse practitioners, physician assistants and a part-time physician. Office hours are Monday through Friday, 8:30 a.m. to 4:30 p.m.
*Note: in some cases, lab tests which need to be sent out for processing by a commercial lab may be billed to a student's insurance
Student Health Insurance
Massachusetts requires all college students have health insurance. Students must enroll in the Stonehill College Student Health Insurance Plan (SHIP) or else waive it by providing proof of comparable health insurance coverage. Information on how to enroll in or waive SHIP will be made available in early July.
