Medical Records Request
Stonehill College Medical Records Request Process for Alumni:
Stonehill College Health Services keeps immunization records for 10 years from your date of graduation.
If you are an Alumni and need a copy of your medical record or immunizations from your time at Stonehill College please fax (508-565-1510) or email (Health-Services@stonehill.edu) our office with a letter indicating the following:
- Name
- Date of Birth (D.O.B)
- Year of Graduation
- Contact Information and Fax # or mailing address of where you would like to have your records sent
- Phone number where you can be reached
- Signature
- Date of Request
Your request will be sent within one week. You may call the Health Services Office at 508-565-1307 with any questions.
External Medical Records Request Process (from an outside facility, including BMC South):
If you were a patient at a medical facility local to Stonehill College and want your medical records from that institution, you will need to contact the institution you received care at directly to inquire about the process to obtain those medical records.
