This form is for adults (staff, faculty, employers, mentors etc.) who have been selected as a reference by a student applying to serve as a Student Leader for the H.O.P.E. Service Immersion Program through the Office of Campus Ministry.

The selection committee is looking for participants who can fulfill the mission of H.O.P.E. while serving as leaders among their peers, in the preparation and re-entry process, as well as during the week of onsite service.

H.O.P.E. stands for Honoring our Neighbors, Organizing for justice, Practicing Peace, and Encountering God. Please take this into consideration when completing this form.
