Health Forms and Insurance
Welcome accepted graduate students! We wish you a happy and healthy graduate student experience at Stonehill. Please review the Health Services required documents.
All new students must submit up-to-date immunization documentation via the College’s Student Health Portal as soon as possible. In order to be in compliance with state laws for incoming graduate students, please contact your primary care provider as soon as possible to request your immunization record. Because of COVID-19, many have limited office hours and staff, and as a result, obtaining records may not be as timely as in the past.
How to Submit Required Health Forms
Please submit all immunization records and health forms at once, if possible, through the College's Student Health Portal.
Your credentials for the Student Health Portal are the same username and password you use to access myHill, eLearn and other College resources.
Required Documents for Graduate Students
Please submit your documents 30 days before the start of your program or if this window has passed then submit within 7-10 days of submitting your enrollment deposit. Students who are admitted to the program after the official enrollment deadline should coordinate with the Office of Health Services to ensure the required health forms are submitted before the start of classes.
Per state law, all students must complete the Tuberculosis Risk Questionnaire. The questionnaire can be found in the College’s Student Health Portal. If you screen as high risk, your PCP must complete the Medical Evaluation for Latent Tuberculosis Infection form, which is also available in the Health Portal.
The Health Report Form is the required intake form for the Office of Health Services. It asks for your consent for medical care at Stonehill College, as well as questions about your personal health history, medications you are taking, and allergies you might have, and the health history of your family. The form also asks for your consent for medical care at Stonehill College. Please find the Health Report Form in the College’s Student Health Portal.
One dose of the Meningitis vaccine (Men ACWY or MCV4) is required for all full-time students. The vaccine must have been received on or after the student’s 16th birthday. The law provides an exemption for students who sign a waiver that reviews the dangers of meningococcal disease and indicates that the vaccination has been declined. Please note, if a student is under 18 years of age, a parent or legal guardian must be given a copy of this documentation and must sign the waiver.
The Meningitis Waiver should be filled out only if the student declines receiving the vaccine and after reviewing the waiver information.
Immunizations
Massachusetts requires all students taking at least 9 credits in fall and spring semesters to fulfill the state’s immunization requirements. For questions, please contact Health Services.
In accordance with Massachusetts School Immunization Requirements, (Law 105 CMR 220.00), Stonehill College requires that all full-time students receive the following immunizations:
- MMR (Measles, Mumps, Rubella)
- Tdap (Tetanus, Diptheria, Pertussis) booster
- Hepatitis B
- Varicella (chicken pox - proof of vaccine or disease)
- Meningitis (vaccine that covers four of the five serogroups of meningitis- strains "A, C, W, Y"
- Tuberculosis Risk Questionnaire (and subsequent testing if high risk)
The College is requiring that all students and employees be vaccinated for COVID-19 prior to the start of the fall semester. We reached this decision after consulting with medical and health officials, reviewing public health guidelines, and monitoring best practices at other colleges and universities. Our goal is to return to our pre-pandemic routines of full classrooms and residence halls, athletic competition, social gatherings, and in-person dining with minimal restrictions. As such, we believe that the vaccination represents our best way to make this all a reality.
- Hepatitis A
- HPV/Gardasil Vaccine (females and males)
- Meningitis Serogroup B Vaccine*
- *We advise students to receive this important vaccine that covers the "B" serogroup of meningitis for full protection against all of the serogroups of meningitis. The Meningitis B vaccine is a series of two vaccines.
Health Insurance
Massachusetts State Law requires all college student taking nine (9) or more credits to have health insurance. Students must be enrolled in a qualifying insurance plan and provide proof of enrollment to the College. Online materials describing 2020-2021 plan details will be available on the Stonehill Student Accounts webpage in mid-June.
How to Waive/Enroll in SHIP
Stonehill College bills all students for the SHIP unless the online waiver process is completed. You will have access to a link on the Stonehill Student Accounts webpage, which describes waive/enroll options and instructions to accept the SHIP, or provide proof of comparable coverage to waive the SHIP. If your current health insurance plan is billable in Massachusetts and comparable to the SHIP, and you are not an International student, you are not required to purchase the SHIP and may waive coverage.

The Office of Health Services offers confidential medical care to all full-time Stonehill students. Its staff includes nurse practitioners, physician assistants and a part-time physician.
