Travel Notification & Transportation Request Form

Instructions

Faculty must provide notification prior to traveling off campus with students by completing the form below. Once the form is submitted, requests will be reviewed.

Once the request has been approved, if a rental vehicle is required, the rental of the most appropriate vehicle(s) will be arranged.

This form should be completed NO LESS than FOUR weeks prior to the date of the trip.

Due to driver shortage and a limited number of school buses available, we will not fulfill requests less than four weeks prior.
