Travel Notification & Transportation Request Form
Instructions
Faculty members must provide notification prior to traveling off campus with students by completing the form below. Once the form is submitted, the request will be reviewed.
When a request is approved, if transportation is required, the most appropriate vehicle will be arranged (e.g., Athletics 12-person van, school bus rental, etc.). If entrance fees or tickets are needed, the purchase can be coordinated with the faculty member.
This form should be completed NO LESS than FOUR weeks prior to the date of the trip.
Due to a driver shortage and the limited number of school buses available, we will not fulfill requests less than four weeks prior.
