Travel Notification & Vehicle Needs Form

Instructions

Faculty must provide notification prior to traveling off campus with students by completing the form below. Once the form is submitted, requests will be reviewed by Craig Kelley, Associate Provost for Academic Achievement, and/or Liz Chase, Assistant Dean of General Education as well as Patrick Keaney, Assistant Director of Student Engagement.

 

If a rental vehicle is required, the rental of the most appropriate vehicle(s) will be arranged by Patty Mead, Program Coordinator, once the request has been approved.

 

This form should be completed NO LESS than three weeks prior to the date of the trip.
