Request Forms
In order to work more efficiently and to set priorities, the Office of Communications and Media Relations (CMR) has established forms for publicity/public relations and hometown newspaper requests. This process helps us to determine which requests we can address and what resources, if any, we can provide.
Please note that request forms should be completed and submitted at least two weeks (14 days) prior to the request due date. Depending on the depth, or complexity of the request, more lead time may be necessary.
