Please fill out the records form below and send it to Liz Greco in the General Counsel's Office or egreco@stonehill.edu. The General Counsel will then sign off on the form and take a copy and return the original form to you for your keeping.
Please do not destroy anything until you have recieved the form back signed by the General Counsel.
