For documentation purposes, faculty must provide notification prior to traveling off campus with students by completing the form below. Once the form is submitted, a curricular request will be reviewed by Craig Almeida, Dean of Academic Achievement, and/or Todd Gernes, Assistant Dean of General Education, whereas a co-curricular request will be reviewed by Patrick Keaney, Assistant Director of Student Activities.
If a rental vehicle is required, the rental of the most appropriate vehicle(s) will be arranged by Patty Mead, Administrative Assistant, once the request has been approved. Requests for vehicle rentals should be made NO LESS than three weeks prior to the date of the trip.
